Attributed Patient Engagement
Zus is a powerful tool for engaging newly attributed patients. There are several ways that Zus can help you reach the patients that need your care most quickly and effectively.
- Get the contacts right: Payers and other partners may not have a patients new phone, email, or address. If caregivers are involved, this can be even more challenging to identify. Zus makes it easy to find this information in the demographics section of the ZAP, or via our Data Marts query for Demographics.
- Leverage clinical context: Review the ZAP to understand what might be top of mind for the patient. Might she need a prescription refilled? Have a recent specialist visit with a new diagnosis? Meeting patients with context and empathy can be immediately impactful.
- Timing is everything: Outreach when patients need help the most. Pay close attention to hospitalizations or other events that may indicate rising risk and need for support.
- Right-size to needs: Is a patient very complex with rising risk? It may be critical to deploy "on the ground" resources to support them and get them engaged quickly. Is a patient stable and may need lighter support? Consider less intensive methods. Zus Data Marts can help you quickly analyze your new population.
